Have you extracted the zip to your ipod yet?  If not, just unzip it to the root of your ipod.  Doing so should place all of the files in their proper locations.

Go to the main menu and select settings.  Then select the bottom menu option, "Browse Themes."  Select the theme you just downloaded, and bammm, your new theme is loaded.  Enjoy!

Here are those instructions in the manual: http://download.rockbox.org/manual/rockbox-ipodvideo/rockbox-buildch5.html#x8-830005.7.4

Please help! I have the same problem, except I have a 5g iPod and I am using a iBook G4.  Also, I can't get the fonts to work.  I extracted both the fonts and the themes to the root, but they don't show up or work.  By the way, when I extracted the Rockbox firmware no .rockbox folder appeared, yet it still worked.  I assume it is hidden.  Except, since it is hidden, I cannot put anything in the .rockbox folder.  Am I the only one with this problem, or does everyone with a Mac have the same problem?  Please help!

When dealing with Rockbox, whether upgrading the build, bootloader, or theme, it's best to do the following so that you can see everything you need to work with.

Copy and pasted from the Gigabeat Port Wiki:
* (For Windows) In Windows Explorer, navigate to the "Tools" menu -> select "Folder Options."
* Click the "View" tab.
   * Check "Show hidden Files and Folders."
   * UNcheck "Hide Extensions for known file types"
   * UNcheck "Hide protected operating system files."
* Click "Apply."
* Click "Apply to All Folders."

The only folders pertaining to Rockbox that should be in the root of your player is the .rockbox folder. Music and Playlists may also be there along with any other folder that you may have created, but the one important to how Rockbox works is the .rockbox folder.  If you see anything outside of that folder, the extraction didn't go as planned and you should move those folders back into the .rockbox folder.  (ie folders named themes, wps, fonts, background, etc.)  Please make sure you agree to overwrite if prompted to  when you copy or extract things into the .rockbox folder.
