This doesn't happen all the time but occasionally. Music in a playlist doesn't always play from the begging but at some point in the middle. When that happens I simply push the skip back button and the music plays from the begging. Not a clue what's going on.

Do you have the Database initialised?

This sounds rather like you may have mistakenly (or deliberately) activated Automatic Resume:

Settings -> General Settings -> Automatic Resume
