There are many (free) programs to generate and manage playlists (actually to manage your music), that Rockbox can play without any further modification. That's true even for automatic & smart playlists generating. You can even convert your current itunes playlist, you won't lose them.

I bought an iPod Video specifically to put Rockbox on it. In the 4 years I've owned it it's been in the original firmware a half dozen times or so and each time I have to tell it I speak English. I've never listened to music, watched a video, or done anything else with it. In fact, I think that most of the times I went into the original firmware was to show a friend that - indeed - you still could.

So, to answer your question, I've never done anything with iTunes or the iPod's database. :)

I bought my 5G before Rockbox had audio support for it, but with the intention of migrating to Rockbox ASAP.  Even back in the early days when battery life was ~50 of Apple's, folder structure support was a most compelling feature for me.
