If it's CC by SA and posted on the wiki, it can most certainly be replaced.
Since CC by SA allows modification, and replacing it is a form of modification, that's more or less all there is to it. This doesn't take into account whether or not one *should* replace another person's work, but simply whether they can.

A good idea might be to have a bug-fix for an appropriately licensed theme posted in the same section with a different file name (so as not to overwrite the original) and a different download link alongside the link to the original, that way the original is still available.
If the theme explicitly states the original author is no longer maintaining the theme, however, I would think it acceptable to overwrite the original theme and the download link with the bug-fix...

I doubt many people that stop maintaining themes will take the time to edit the themes to state that.
As a note, I wasn't commenting on the rights or wrongs, just pointing out that at least a small point of the licence is to allow other people to update (for example) in the event that the original author no longer is.

Well, that is true... I've stopped maintaining some of my older themes and bothered to explicitly state such, but then again I'm still around making newer themes anyway...

I think it was I who added to the WPS galleries "Please honour the original author's intent and don't upload a "fix" which changes features or behavior. If you wish to modify a theme in this manner rerelease it under a new name. " when the graveyards were split.
If it is felt such a request is not enough, or that some "courtesies" for the author should be requested, perhaps this should be discussed. That said, I do believe modification ("replacement") is obviously allowed by the CC license, and the wiki can clearly be considered a crude CVS.
