Thank you very much for file skipping/buffer/response fixes!!!!!

Thank you very, very much for the recent buffering fixes that have fixed the file skipping issue I was experiencing earlier this year.  Auto change dir is working as well.  It's a preliminary report, but these changes look fantastic and file skipping forward and backward may even be faster than ever before.

I think (apologies if I'm incorrect) Nico_P was primarily responsible for these changes so a special thanks to him.

r17157 on iPod 5.5G 80GB with 12000+ .ogg files at -q7 -q8 and some at -q10, dircache ON.
